A DAY IN THE LIFE OF A RESOURCE SPECIALIST In this position, you will provide person-centered care coordination with older Minnesotans and caregivers over the phone and in-person. You will be providing options counseling, advocacy, resource location assistance and education. You will research home and community-based service options and assist clients as they navigate systems and comply with specific protocols and documentation requirements. QUALIFICATIONS 2 years of experience with case management or care coordination in a long-term care, hospital, or community setting Compliance with all state licensure requirements Bachelor's degree from an accredited college or university in a human services-related field. Social work and nursing degrees strongly preferred. Reliable transportation Heart for working with the elderly Do you have excellent communication and interpersonal skills? WORK SCHEDULE FOR RESOURCE SPECIALIST This position works Monday - Friday, 8:00 AM - 4:30 PM, with two to three in-office days per week and theremaining time spent working from home.
