together. The Community Health Worker is responsible for
assessment, planning and implementing care strategies that are
individualized by patients and directed toward the most
appropriate, least restrictive level of care. They also Identify
and initiate referrals for social service programs; including
financial, psychosocial, community and state supportive services,
and manage the care plan throughout the continuum of care as a
single point of contact for the member. As a Community Health
Worker (CHW), you will act with Medicaid members to ensure
appropriate care is accessed as well as to provide home and social
assessments and member education. The Community Health Worker also
addresses social determinants of health such as transportation,
housing, and food access. Together, we're removing the barriers

position is full-time, Monday - Friday. Employees are required to
have flexibility to work any of our 8-hour shift schedules during
our normal business hours of 8:00am - 5:00pm. It may be necessary,
given the business need, to work occasional overtime. This position
is a field-based position with a home-based office. You will work
from home when not in the field. Location: Eden Prairie and
surrounding communities in Minnesota. If you reside in Eden
Prairie, Minnesota (Twin Cities), you'll enjoy the flexibility to
telecommute* as you take on some tough challenges. Local travel up
to 25% and mileage is reimbursed at current government rate.
Primary Responsibilities: - Create a positive experience and
relationship with the members - Help members set goals and develop
a care plan to achieve those goals - Proactively engage the members
to manage their own health and healthcare - Support the members to
improve their wellbeing by staying out of the hospital, and attend
regular visits to their primary physician - As needed, help the
member engage with mental health and substance use treatment -
Support the member to ensure pick-up of their prescriptions -
Provide member education on community resources and benefits -
Conduct post discharge activities from hospital facility and
support connection to social services - Support members to engage
in work or volunteer activities, if desired, and develop stronger
social support through deeper connections with friends, family, and
their community - Partner with care team (community, providers,
internal staff) - Knowledge and continued learning of community
cultures and values - May conduct Health Risk Assessments (HRA) if

in. Required Qualifications: - High School Diploma/GED (or higher)
- 1 years of computer proficiency experience including MS Word,
Excel and Outlook - 6 months of field-based experience of community
advocacy - Access to a designated quiet workspace in your home
(separated from non-workspace areas) with the ability to secure
Protected Health Information (PHI) - Ability to travel locally
approximately 25% of the time up to 100 miles round trip - Ability
to work full-time, Monday - Friday. Employees are required to have

resided within the local community for 2 years - Must reside within
a commutable distance Eden Pairie, MN, and surrounding communities
- Access to reliable transportation that will enable you to travel
to client and/or patient sites within a designated area, with a
current and non-restricted state of Minnesota driver's license and
state-required insurance Preferred Qualifications: - Knowledge
and/or experience with behavioral health or substance use disorders
- Care management experience - CHW Accreditation - Experience
working in Managed Care - Knowledge of Medicaid and/or Medicare
population - Knowledge of culture and values of community -
Familiarity with the resources available in the community -
Bilingual English/Spanish Proficiency Soft Skills: - Ability to
work independently and maintain good judgment and accountability -
Demonstrated ability to work well with health care providers -
Strong organizational and time management skills - Ability to
multi-task and prioritize tasks to meet all deadlines - Ability to
work well under pressure in a fast-paced environment - Excellent
verbal and written communication skills; ability to speak clearly
and concisely, conveying information in a manner that others can
understand, as well as ability to understand and interpret
