Every service we offer is a response to the needs and goals of our clients and their families, and our efforts as a company are inspired by the desire to explore possibilities for meeting those needs.We are seeking a full-time Admissions Registered Nurse (RN) who will perform Start of Cares (SOC), Resumption of Cares (ROC) and conduct in-home assessments and re-evaluations, assist in preparing care plans, perform medication set-ups, supervise HHA's and CNA's. The SOC RN is responsible for evaluating and opening the case for home health patients. The SOC RN identifies and creates the home health care plan for admission under a home health episode/authorization while ensuring to educate patients, families, caregivers, and other providers to safely perform care.Job Duties:Conduct initial home health care OASIS assessments for comprehensive care, including Starts of Care, Resumptions of Care, and Recertifications.Develop a plan of care based on physician orders and initial client assessment and continue to monitor, revise, and update based on ongoing assessments or changes of conditionFacilitate referrals to interdisciplinary team (Physical Therapy, Occupational Therapy, Aroma Therapy, Speech Therapy and Falls Prevention)Perform medication set-ups as needed or required under the direction of the Clinical Manager and CareAparent schedulingOversee and supervise Licensed Practical Nurse (LPN) visits and ensure plan of care is being followedCoordinate, oversee, and supervise on-site CNA / PCA / HHA visits and ensure plan of care is being followedReport changes in client condition to physicians and Clinical ManagerPrepare appropriate medical documentation of client assessments and visits using CareAparent EMR platformsPerform other nursing functions as may fall under state and federal nursing guidelines, CMS, and the Joint CommissionAble to work on call for rotating evening, weekends, and holidaysPerform other skilled nursing visits as directed by Clinical ManagerMaintain RN license annuallyQualifications:Graduation from an accredited and approved nursing program; BSN preferredMinimum of one (1) year of relevant home care or assisted living experienceValid Registered Nurse license in the state of Minnesota.Valid drivers license with an insured, reliable vehicle.Knowledge regarding quality improvement and home health standards of care.Knowledge regarding the impact of acute and chronic illness on the lives of patients and families in the community.CareAparent is an Equal Opportunity Employer and qualified applicants will not be discriminated against on the basis of race, color, religion, gender, national origin, disability or veteran status.CareAparent participates in the federal E-Verify program.by Jobble
